Yes there have been many games that have been graded 100, all of them a pretty much newer gen games such as PS3, WII, xbox, GC, 360 etc.
They get the grade by having a factory sealed box of the game which will have 10 or so games in it, the box it sent to VGA so is untouched.
Of course you could get the grade if the item has been looked after extremely well but unlikely to get 100.
If a seller has more than 1 VGA 100 item then it's someone who works at gamstop or other retailer and distributor and is pure profiteering.
